Fee Schedule Hormone Management by Pellet Therapy
- Candidates For Therapy
- Women: perimenopause or menopause
- 35 years or older, or with a history of premature ovarian failure or surgical/treatment-induced menopause
- Perimenopausal women who are sexually active with a male partner must have a reliable contraceptive method in place (e.g., tubal ligation/vasectomy (partner), Essure, IUD, hormonal contraception) and actively prevent future pregnancy.
- Prior breast cancer patients should be cleared by their provider/oncologist for treatment.
- Men:
- 40 years or older
- No desire for future fertility
- Prior prostate or breast cancer: must have stable/treated disease and cleared by their provider/oncologist.
- Not on blood thinner
- Further exclusions may be identified at time of consultation.
- Women: perimenopause or menopause
- Initial comprehensive in-office consultation, follow-up lab review + extensive lab panel and supplement recommendations: $850
- Initial consultation: 1 hour office-visit + 45–60 min follow-up lab review
- Comprehensive lab panel: Detailed laboratory evaluation to include sex hormones, extended thyroid panel, complete metabolic panel, complete blood count, marker for carbohydrate metabolism, inflammatory marker, nutritional markers (vit D, B12, ferritin, homocysteine), fasting insulin/glucose/lipid panel ($2500 value) and supplement recommendations.
- Essential only evaluation $550: one-time consultation, essential hormones-only labs, lab review (1hr appt). Labs must be drawn at least 1 week prior to consult; pellet therapy can be started same day (upon request).

- Men $1980: includes 2 insertions (approximately every 4–6 months) and all monitoring labs(3 lab sets)
- Single insertion/lab price: $1040
- Women $1940: includes 4 insertions (approximately every 3–4 months) and all monitoring labs (up to 4 lab sets)
- Single insertion/lab price: $510
- Monthly payment plans available with automated payment; 5% service charge applied.
- Men $1980: includes 2 insertions (approximately every 4–6 months) and all monitoring labs(3 lab sets)
Payment Information
- Payment is required in full for all services rendered on the day of service. Payment by credit card, cash or check is accepted.
- BioAge Health is not contracted with any insurance company and does not submit claims, complete forms or otherwise communicates with your insurance in any manner. It is the patient’s responsibility to submit a claim to my insurance company for possible reimbursement if desired. I (the patient) understand that my insurance company may not reimburse me, depending on my coverage. I understand that BioAge Health, PLLC is also not a Medicare provider and services provided by BioAge Health, PLLC are not covered by Medicare. I acknowledge that BioAge Health, PLLC has no contracts with any insurance companies and is not contractually obligated to pre-certify treatment with my insurance company or answer letters of appeal.
